Much depends upon how each viewer chooses to respond to the image. If you consider the image to be either sexual, exploitative or both, that is entirely in your own head.
The thing that most definitely is terrible about the BF album cover is the montage work. The joins are glaringly obvious. By way of comparison, think of the front cover of Ummagumma by Pink Floyd. Another razor blade montage job. You know where the joins must be but they have been so well executed that they are almost undetectable.
My own nomination for terrible album cover design execution is Rush - Hemispheres.

The Scorpions cover goes above and beyond to indecency and offensiveness.

Great album, but terrible cover IMO.
I'll nominate the British cover of Electric Ladyland. All adults, but it still seemed very tacky and the manipulation of the image to slightly distort the women made it worse. Hendrix hated it apparently.
