Can anyone recommend a practice diary?

 I've had a look on Amazon and there are quite a few, but none of them show what the pages look like.

I don't do it for guitar, but I do try and keep track of some aspects of my clarinet practice. It's mainly to make sure I'm not subconsciously avoiding anything, or if I want to make a note of something to practice next time. I use an A5 week-per-view diary and find the paragraph-sized gaps for each day give enough space to put some notes down.
