I've used Earlpilanz on eBay for 'no pot holes' pickguards (my Strats only have two knobs) and they were okay. I think the one I bought from Custom World Guitar Parts in the Netherlands was a little better fit and finish. Neither do single ply ones as stock, though as @paulnb57 points out, Earlpilanz does do some custom work.
The one I got from CWGP is this one (they do other colour/routing variants, but all multiply).

I paid CWGP for a special tele deluxe scratch plate and after weeks of wrangling, never got it or my money back. If they actually have the one in stock that you need, great.
I've had two specials made by Jacks Instruments and if you have spent a lot on a fancy pants partscaster. A lot of people say they can make customs scratch plates, but Jack actually does.
