The only time I can think of I've ever lost my temper on the phone and shouted at the person at the other end was when some little office nerk at the DVLA told me I wasn't getting my road tax refund because I'd spelt my name wrong on the form :x. (So it didn't agree with the vehicle registration docs.)

When I asked her to read it out from the computer screen they had of course transcribed it from the form using the same misspelling that about 95% of people do.
