    sometimes you see actual dents in frets, usually from  something hitting the strings against the fret.  they should always be taken care of as they can catch the string more than natural wear

    I've got exactly that issue with a 14th fret on the D string. I think something dinged the guitar right there and left a slight indent.  The object hits the string and dents the fret in this scenario just as WezV says - not something you'd actually think about until it happens. 

    Nothing like as deep as WezV's pick - but really annoying. Discovered as a pulling the string up or down on that fret changed from a burning sound to a pure note either side. Fortunately, it's quite a bit up the board and I can live with it - but I will have to get this polished out.
