I bet Bright Onion would make you one too if you asked nicely.
It would be sensible to have a MIDI thru box tucked away under the board too- not all MIDI pedals do MIDI thru reliably.

Seconded - this is what I have too, and allows for 1 MIDI in and 5 MIDI out's
I don't think this is quite what the OP wants- he's on about sending MIDI from his pedalboard to his amp, and needs somewhere to conveniently connect the cable.
A MIDI Thru will probably be necessary to control multiple pedals on the board, but if you mount it at the edge of your board to make it easier to connect to the amp, you end up with all the other MIDI outs hanging out the side of the board too...

I've got one of these 3-way ones which connects a set of PK-5A pedals to a Minitaur, a Roland JV-1010, and a Peavey Spectrum (on MIDI channels 1, 2, and 3 respectively).

The opposite is a MIDI Merge box, which takes several controllers as input and pushes them down a single output.

There are some boxes which combine the functionality, including some which have two inputs which can be switched to either of four outputs.
